My heart it breaks

When I can’t do anything

To make you smile

To make you laugh

My tears they fall for you

When you are strong

When you wish not to show

The sorrows of your day

My smile disappears

When you can’t smile back

My greatest fear

Is for that smile

To be gone forever

Life isn’t worth living

If I can’t see you smile

If I can’t hear you laugh

I would go to extremes

Just to see you smirk

Just to hear you chuckle

Just to know that I

Could for just a second

Distract you from your sorrows

My only suggestion for the line "My smile disappears" to add a 'it' so it would become "My smile, It Disappears" Because you had it for the first two lines like that. It would add to the flow a bit. Or I suppose you could take out it, or they to the other two lines.
Otherwise I think its a great Poem
